Below is a current list of what services we’re offering through Glow Health Services with Rondal Zapf, CRNP, Holly Williams, RN and Keana Cozatt, RN.
Botox/Jeuveau:
Must Be 18 Years of Age
Initial Consult: This is a 30 minute appointment and includes consultation and first dose if wanted. $100 deposit is required for this service to be booked. Credit card on file is required for Botox appts.
Re-check: This is an appointment two weeks after receiving botox to just check to see how everything went and to get pictures. This appointment is required, and cannot be pushed out any longer than 3 weeks.
Follow-Up: This is a 30 minute appointment scheduled to receive your next session of Botox 3 months after your first one.
Underarms: This is a 30 minute service. Each arm gets 45 units. This makes each unit $9.75 each. Will last 6 months. Requires follow-up.
Underarm Re-Check: This 15 min appointment is only needed if you feel that you are not seeing enough improvement. Cannot be pushed out any longer than 3 weeks.
Underarm Follow-Up: This is a 30 min appointment scheduled to receive your next session of Botox 6 months after their first one.
Lipostat Shots
Must be 18 years of age, or 12 with PCP clearance letter
(Ingredients: B-12, Methionine, Inositol, Choline, B=helps boost energy, fuels metabolism, and balance your overall well being)
$20 per shot, or 4/$68
Initial Consult Required: During this 15 mInute appointment, you will meet with Rondal and make sure you are suited for this shot. You will discuss if you want to get one dose or if you want to start on a monthly package. The monthly package saves you 15%!
Weight Loss Plans
Must be 18 years of age, or 12 with PCP clearance letter
To start our weight loss plans, you do have to go through a consultation process. It is a series of 3 consults which can be done in 2 or 3 visits. This is a $125 charge and it includes:
- Weight Loss Consult #1: 30 minute consult with Rondal, where you will be given lab slip for bloodwork. Paperwork will be sent to you when you schedule your consult and must be filled out 48 hours prior to your appointment so it can be reviewed. The $125 fee is due at this visit.
- Weight Loss Consult #2: 30 mins (can be the same day) with Holly or Keana. You will be set up with a nutrition tracking service or app, and educated on basic nutrition. Will e-mail you or provide you with paper copies of everything to help you along your journey. Weight, measurements & photos will also be taken at this visit.
- Weight Loss Consult #3: 30 mins with Rondal. At this appointment you will go over lab work and personalize a plan just for you. You will receive pricing at this appointment for any injections or medications you will be getting. If you decide to start our weight loss program at this appointment, you will pay for the monthly fee which will include your monthly program fee and any injections/medications you will need for the month.
- What’s included in the $100 monthly program fee:
- 2 COMPLIMENTARY check in appts with a RN with the first month. After the first month you will only come in once a month.
- The first 2 weeks, you will come in weekly to meet with an RN for 30 mins to do weight, measurements & pictures and check in on how you are feeling. Talk about eating habits and water intake. Then you will come back on the 4th week to do the same thing. Once you go to monthly weigh ins, you can add one 15 min check in appointment free of charge each month for any questions if needed.
